When submitting a request for number transfer to O2, you will need the following documents.
Person - Czech citizen
- valid citizenship identification card and one more piece of photo ID issued in the Czech Republic (e.g. passport, driver's license, firearm license)
Person - entrepreneur
- valid citizenship identification card and one more piece of photo ID issued in the Czech Republic (e.g. passport, driver's license, firearm license)
- trade license
Legal entity - executive officer
- valid citizenship identification card and one more piece of photo ID issued in the Czech Republic (e.g. passport, driver's license, firearm license)
- copy of your record at the commercial registry (not older than six months)
Legal entity - a representative other than an executive officer
- valid citizenship identification card and one more piece of photo ID issued in the Czech Republic (e.g. passport, driver's license, firearm license)
- copy of your record at the commercial registry (not older than six months)
- notarized power of attorney
