Printed phone directory
General information
In accordance with a resolution of the Czech Telecommunication Office dated December 21, 2005, Telefónica O2 Czech Republic, a.s. won a tender for the supply of individual services within a universal service - Issuing Telephone Directories and Supply of Information Services regarding Telephone Numbers. Both services shall be ensured in accordance with the new Law on Electronic Communications. The contractual publisher of telephone directories for Telefónica O2 Czech Republic is MEDIATEL, spol. s r.o.
The telephone directory contains the subscriber telephone numbers of all operators that provide a publicly accessible telephone service and which duly submitted their subscriber data to Telefónica O2 Czech Republic, except for data regarding subscribers who refused its publication in the telephone directory.
The subscribers of all operators that provide a publicly accessible telephone service are entitled to receive the telephone directory. A fixed telephone line subscriber shall receive the telephone directory for the relevant defined territory (region) or district that is relevant to the location of their subscriber line free of charge. The address of the line's location is stated in the list according to the Regional Identification Address Register (UIR-ADR) that is administered by the Ministry of Labor and Social Affairs. A mobile telephone number subscriber may receive the telephone directory for the territory according to the selection in the form mentioned below free of charge.

Voucher
Upon the presentation of the voucher at the distribution point (a list of distribution points including the distribution timetable is presented below) each subscriber shall receive the defined quantity of telephone directories according to the number of fixed telephone lines or mobile telephone numbers entered in this form.
If the subscriber has telephone lines connected to an extension in the exchange we recommend calling the Infoline at the phone number 1180 to have the voucher sent. An operator will fill in an electronic form with the subscriber, print the voucher and send it to the subscriber by post.
You can also obtain a voucher in one of the following ways:
- The subscriber will receive a voucher together with their statement for electronic communication services
- The subscriber will visit a O2 store where the staff will fill in the electronic form with them and print the voucher.
- The subscriber will call the Infoline at the phone number 1180, where an operator will fill in the electronic form with them, print the voucher, and send it to the subscriber by post.
The subscriber can also receive the telephone directory without the voucher in O2 stores (which are presented in the list of distribution points) and at MEDIATEL, spol. s r.o., Thámova 16, Prague 8, where, on the basis of information provided by the subscriber, staff will fill in the electronic form and issue the telephone directory to the subscriber. It is possible to buy telephone directories for all regions of the Czech Republic at MEDIATEL, spol. s r.o., Thámova 16, 186 00 Prague 8 throughout the year.
